Official Title: Artificial Intelligence (AI) Skilled Nursing Facilities (SNF) Discharge QI Project
Brief Summary: This study evaluates an AI-supported workflow tool designed to improve discharge planning for hospitalized medicine patients discharged to skilled nursing facilities (SNFs). The intervention displays a patient list column showing SNF discharge risk as High, Intermediate, or Low, along with a hover bubble containing AI-generated summaries of the history and physical note to provide relevant clinical context. The control condition hides the column value.
Conditions: Skilled Nursing Facility Patients
Interventions

Ages Eligible for Study: Adult (18-64)Older Adult (65+)
Sexes Eligible for Study: All
Accepts Healthy Volunteers?: No
Eligibility Criteria: Inclusion Criteria:

Adult medicine inpatient admission

Discharged to a skilled nursing facility

Admission occurs during the study period

Exclusion Criteria:

Age younger than 18 years

Not admitted to a medicine inpatient service

Not discharged to a skilled nursing facility

Admission occurs outside the study period
