Patient at Gottschalk Medical Plaza Integrative Medical Clinic
Ability to complete survey

Exclusion Criteria:

Patient less than 41 years of age
Inability to speak English
Patient unable to provide informed consent
Patient not being seen at Gottschalk medical Plaza Integrative Medical Clinic
Incomplete survey
